School-wide Suicide Screening alternating grades on different days.

School announces screening days and each student is given a preliminary questionnaire or survey. These assessments may cover various areas such as anxiety, depression, self-esteem, stress, or social-emotional functioning and suicidal risk. Those that indicate any suicidal risk or mental health risk are then selected for follow up suicide screening with a trained screening professional. The professional determines low/medium/ high risk and if safety planning is required.

Requires:

  • All students take part in preliminary questionnaire or survey. Families are alerted and can choose to opt out their student from screening.

  • Trained screening professionals; trained in safety planning
