Training-gated
Thirteen questions. Ten to twelve minutes with a counsellor.
What it asks, and in what order
Who you are. Name, date of birth, email, whether emailing is safe, cell number.
When to open this. Warning signs, as a checkbox list with room for their own words.
Three levels of what helps.
- When I am feeling down
- When thoughts of suicide start
- When it feels like too much
People. Up to three adults with real contact information, and whether there is an adult at home they would want called.
Numbers that are always answered. Printed on the form, plus the school contact by name.
What I am staying for.
Who is checking in, and when. A name, a day, a time.
Why the three levels matter
They are the clinical spine of the instrument. Coping on your own, then getting your head somewhere else, then reaching for a person. A student in the third state cannot use a strategy that works in the first.
Do not collapse them. Without the ladder this becomes a list of hobbies.
Why checkboxes, with room to write
A student who has just had a hard conversation will stall in front of an empty box. Ticking six things takes twenty seconds and gives you something to ask about.
Every checkbox question has an Other field. That is where their own words go, and their words are the part they will use.
✓ Take Action. When they tick something, ask about it. "You picked being outside. Where do you go?" The answer that goes in the Other field is better than anything on the list.
The email question
The form asks whether it is safe to email them a copy.
That is not a formality. A school account may be visible to district IT. A personal one may be logged in on a family tablet. For a student whose risk is connected to what happens at home, an automatic email is not neutral.
Hesitation on that question is information. Ask who else has access to that inbox. Do not talk them into the email. Printing is always available.
What the student leaves with
A copy of the plan. Emailed or printed, their choice.
The resource list, on paper, theirs to keep. See Module 3.8.
